The Crisis Peer Support Coach assists individuals throughout recovery, helping them gain skills, access resources, and regain the confidence to succeed in society. They will initiate contact with patients in crisis and support them throughout the recovery process by providing guidance, advocation, validation, respect, and empowerment drawing on their lived experiences. This position works in acute settings that address crisis mental health and substance use care between crisis settings as well as in an outreach setting with a multidisciplinary team.

Responsible for oversight and direction for nurses (RNs), Patient Navigators (PNs), and Medical Assistants (MAs) for the Health Care District’s Primary Care Clinics. Provides clinical guidance and training to RNs and MAs. Evaluates the effectiveness of the care provided to patients by RNs and MAs. Monitors ongoing RNs and MAs clinical performance. Maintains responsibility and accountability for RNs and MAs to ensure their performed duties are in accordance with established policies, nursing standards, and clinical protocols. Coordinates risk-related activities for the C. L. Brumback Primary Care Clinics.

The Behavioral Health Technician (BHT) is responsible for the well-being of our Clinic patients who struggle with a variety of addiction and behavioral problems. The BHT works closely with patients and medical staff, assisting in the areas of medication observation, and medical appointment scheduling. They observe and document patient behavior, assist in the treatments, and maintain a safe, secure, and supportive clinic environment. The BHT role involves observing and charting patients, collecting lab samples, preparing medical data for clinical staff, and providing education for prescriptions and medication management. The role requires knowledge of addiction and behavioral health medications, attention to detail, and accurate documentation abilities. BHTs are required to uphold ethical and policy stipulations in their role. This position provides administrative support to all clinic staff and its departments, with emphasis on registration, cash reconciliation, clinic work flow, patient cycle time, customer service, management of appointment schedule, and staff work schedule.

The Provider Enrollment Specialist is responsible for coordinating all necessary provider enrollment applications for billing to various carriers.  This position is also responsible for ensuring all applicable documents have been sent/received in a timely manner to the authorized signer and preparing/submitting a completed payer application.  Responsibilities also include completing all CAQH re-attestations, online application and re-enrollment applications.

The Medical Assistant performs assigned administrative and clinical duties under the direction of the Practice Management, Senior Clinical Nurse and Physician. Administrative duties may include scheduling appointmentsand maintaining medical records. Clinical duties may include taking and recording vital signs, social determinants of health (SDOH) and medical histories, preparing patients for examination and administering medications as directed by physician. About the C. The BHC is a licensed clinical psychologist or LMHC or LCSW who adheres to the highest standards of evidence based medical practice, ethics, and professionalism at all times.  The BHC provides direct behavioral health integration to children, adolescents, adults and families in order to improve psychosocial function in a primary care setting.

This position collaborates with the Senior Leadership team on strategic planning, goal setting, department evaluations, and the implementation of strategic initiatives for the respective department. The Quality Manager works in partnership with the CL Brumback Primary Care Clinics administrative and medical staff leadership to implement and maintain quality initiatives, reporting, and policies, and procedures. This position is responsible for providing oversight, as well as coordinating and evaluating all facets of the CL Brumback Primary Care Clinics quality management and patient safety activities.

The Performance Excellence Facilitator is responsible for the implementation and delivery of the Healthcare District Palm Beach County Performance Excellence Program under the direction of the Assistant Vice President Organization Transformation.  Performance Excellence Facilitators will partner with Performance Excellence Business Partners and  HCDPBC Managers and Associates to identify and improve operational efficiencies by measurable results by reducing variation and waste, and improving process flow, speed, and quality and will provide on-going support to teams to become Continuous Improvement practitioners.  The goal of the Performance Excellence Office is to improve HCDPBC net operating results 3% per year on average, through the implementation of lean management systems, and to enable healthcare delivery re-design from volume to value.  This will be accomplished through the self-directed application of lean concepts and methods, such as Value Stream Maps, 5S and Kaizen Rapid Improvement Events throughout HCDPBC with the management team.
